  • High Risk Clinic – Coronary Artery Disease (CAD)

    GOAL

    To reduce and manage the risk of CAD for patients with modifiable risk factors

    PROGRAM

    A one year program led by a Cardiologist covering all aspects of CAD assessment and management

    COST

    There is no charge for the program

    WHO QUALIFIES

    1. Diabetic adults with at least one additional risk factor (see below) —or—
    2. Adults with a combination of at least two risk factors:
    • a. Uncontrollable risk factors (e.g. age, family history, ethnicity) —and/or—
    • b. Modifiable risk factors (e.g. hypertension, dyslipidemia (cholesterol), obesity, smoking history, diabetes, poor diet, physical inactivity, high stress) —or—
    • c.Stable CAD and/or post intervention (CABG, stent, angioplasty) of one year or more

    BENEFITS

    • Comprehensive assessment and medical, pharmacological and lifestyle management of CAD
    • Proactive prevention and active management of CAD
    • All test results, modifications and interventions are communicated to the Family Physician
    • Family physician has direct and continuous access to the responsible Cardiologist
